Exocrine Pancreatic Insufficiency in simple words is defined as maldigestion in dogs. When a dog shows a dry and dandruffy coat it is surely a telltale sign of Exocrine Pancreatic Insufficiency or EPI. It happens when the animal fails to absorb dietary fats. In major cases, dogs before they turn 4, show signs of this disease. Mostly, German Shepherd dogs and Rough-coated Collies are vulnerable. In some cases, Greyhound has been diagnosed with the juvenile-onset disease. Terriers, Cavalier King Charles Spaniels, Chows Chows, and English Setters have also been reported with EPI.

The test, and the number to ask for

EPI is diagnosed on one blood test, and it is worth knowing its name because a general work-up will not necessarily include it.

The test is serum trypsin-like immunoreactivity, usually written TLI. Merck: “a low serum trypsin-like immunoreactivity (TLI) concentration (≤ 5.5 mcg/L in dogs or ≤ 8.0 mcg/L in cats) is diagnostic” (Merck Veterinary Manual, retrieved 2026-08-12).

Two practical points. Your dog will normally need to be fasted for the sample, so ask before you go. And ask for the actual number rather than “it came back low”, because the value tells your vet how much function is left.

Ask for cobalamin, vitamin B12, and folate at the same time. The reason is in the treatment section below, and it is the single most useful thing on this page.

The picture that should make you ask for the test

The combination matters more than any single sign.

A hungry dog that is losing weight. Merck notes dogs with EPI “commonly have polyphagia,” meaning abnormally increased appetite, alongside “weight loss and loose stools” (retrieved 2026-08-12). Eating more and getting thinner is the signature.

The stools are distinctive. Merck describes them as “pale, loose, and voluminous and may be malodorous” (retrieved 2026-08-12). Voluminous is the word owners recognise immediately: the volume is startling relative to what went in, because most of the food is passing through undigested.

Alongside those, owners often report a dull, poor coat, wind, and a dog that will eat things it never used to, including its own faeces. None of that is misbehaviour. It is a starving dog looking for calories it is not absorbing.

Why it seems to appear overnight

Owners almost always describe EPI as sudden. One month a normal dog, the next a thin dog producing enormous pale stools. There is a reason for that, and it is a single number.

Merck states that clinical signs do not appear until “approximately 90% or more” of exocrine pancreatic function has been lost (Merck Veterinary Manual, retrieved 2026-08-12).

So the pancreas has been quietly failing for a long time while the remaining tissue covered for it. By the time your dog looks ill, the reserve is gone. Nothing happened overnight except crossing a threshold.

That also explains why this is a young dog’s disease in this breed. Merck records that EPI due to pancreatic acinar atrophy “is most frequent in young adult German Shepherd Dogs” (retrieved 2026-08-12). A two-year-old losing condition is exactly the profile, and it is exactly the age at which owners assume it must be something simpler.

Causes that Lead to EPI

Exocrine Pancreatic Insufficiency in German Shepherds

The light pink color glandular organ lying below the stomach has a great role to play in the digestion process in dogs. Placed alongside the duodenum, the pancreas is divided into two different sets of pancreatic tissues called the exocrine and endocrine pancreas. Endocrine and exocrine are responsible for the secretion of blood sugar-regulating hormones(insulin and glucagon) and digestive enzymes (amylase, lipases, trypsin, proteases) respectively. Enzymes originally remain in an inactivated form within special granules present in the exocrine tissue of the pancreas, which is called acinar cells. They pass down into the duodenum, the beginning tract of the small intestine, and kick-start the digestive process once the ground-up food enters the intestine. The food breaks down into tiny molecules which are easy to get absorbed as the digested food travels down the gastrointestinal tract.

Now when there’s a deficiency of this enzyme the food fails to get digested and absorbed. In such a scenario, the dog becomes skinny and its coat loses its shine. Acute enzyme deficiency is accounted to a medical condition called ‘pancreatic acinar atrophy’. The pancreas reflects shrunken lobes in such a situation. In dogs, EPI can also be caused by Chronic Pancreatitis and Pancreatic Cancer which damages the acinar glands. Pancreatic cancer is a rare cause of the disease.

Signs and Symptoms of EPI

The most common symptom of EPI in dogs is ‘cowpie’ stool which is usually orange, gray, or yellow in color. Diarrhea gets followed by rapid weight loss. The pet shows polyphagia which refers to a ravenous appetite. The voluminous amount of stool with greater frequency bothers the animal throughout the day. An instance of gas, burping, and flatulence are often seen in such dogs.

Is there Any Genetic Connection?

There is no genetic basis as such and the mode of inheritance is termed autosomal recessive. It means that the genetic carriers appear absolutely normal.

Diagnosis of EPI

The most confirming blood test that helps in diagnosing EPI is Serum Trypsin-like Immunoreactivity Test (or TLI). An EPI-affected dog will have nil Serum Trypsin-like Immunoreactivity in the bloodstream. Other popular tests include Fecal Protease Test and Fecal Elastase Test.

Effective Ways to Treat EPI Dogs

The best treatment is the inclusion of dietary supplements along with digestive enzymes. These supplements along with the dietary proteins are digested with the help of naturally secreted protein-digesting chemicals. Those which escape the stomach’s acid bath and protein-digesting chemicals go on to stop diarrhea , and in the process, the dog gains weight.

Powdered enzymes also work but not when it is encapsulated in a gel capsule. These include Viokase-V and Pancrezyme. But care should be taken to mix the powder well in the food because it can be abrasive otherwise and can lead to the formation of mouth ulcers. In some cases, the patient might show a good response to an H2 blocker kind of an antacid like famotidine.

Another way out is feeding the pet with raw foods like beef or lamb pancreas. It’s a good alternative for enzyme replacement but the problem lies with bacterial and parasitic contamination. Last but not the least, is feeding the dog with easily-digestible food , food that’s low in fiber and fat.

EPI patients have a massive population of bacteria within the intestine and this leads to Vitamin B-12 or cobalamin deficiency because the bacteria feed on this vitamin. Hence antibiotics or Vitamin B-12 injections along with frequent blood tests are recommended for EPI dogs.

Treatment: what the evidence actually says

This is where a lot of well-meaning advice goes wrong, so here it is straight from the source.

Powder beats tablets. Merck: “powders are more effective than tablets, capsules, and especially enteric-coated products” (retrieved 2026-08-12). Enteric coatings are designed to survive stomach acid and dissolve further down, but dissolving them relies on pancreatic bicarbonate, which is exactly what an EPI dog is short of.

The starting dose. Merck gives it as “1 teaspoon/10 kg body weight should be given in food with each meal” (retrieved 2026-08-12). Every meal, not once a day.

You do not need to pre-mix and wait. This one contradicts a lot of what gets passed around in owner groups. Merck states plainly: “preincubation of the food with pancreatic enzymes or supplementation with bile salts is not necessary” (retrieved 2026-08-12). If sitting the bowl for twenty minutes has become part of your routine, you can stop.

The dose comes down, not up. Merck notes “the dose can be slowly decreased until the lowest effective dose has been reached” (retrieved 2026-08-12). Enzymes are expensive, so this matters. Do it with your vet, not by guesswork.

Diet: Merck recommends “a diet low in insoluble or nonfermentable fiber” (retrieved 2026-08-12). High-fibre “digestive” foods can make things worse here.

The B12 question that changes outcomes

If you read nothing else, read this.

Merck records that “most dogs and almost all cats with EPI are cobalamin deficient,” and that in a study of dogs with EPI, cobalamin deficiency was “the only independent risk factor for poor outcome” (Merck Veterinary Manual, retrieved 2026-08-12).

Read that again. Not the enzyme dose. Not the diet. The B12.

The reason is mechanical: most intrinsic factor, the protein needed to absorb B12, is made by the pancreas. A pancreas that cannot make enzymes generally cannot make enough intrinsic factor either. So a dog can be on the correct enzyme dose, eating well, and still failing to thrive.

Merck gives supplementation as “25 mcg/kg, SC, once weekly,” with oral dosing also described (retrieved 2026-08-12).

So the question to ask, by name: “has my dog’s cobalamin been measured, and does it need supplementing?” If your dog is on enzymes and still not gaining, that is the first thing to check rather than raising the enzyme dose.

What life looks like afterwards

The honest framing is two-sided, and the second half is better than most owners expect.

Merck notes that “recovery is rare”, meaning the pancreas does not regenerate and this is managed rather than cured. But it continues: affected animals “usually gain weight quickly, pass normal stools, and can live a normal life for a normal lifespan” (retrieved 2026-08-12).

That is a genuinely good outcome for a disease that looks devastating at diagnosis. The cost is a scoop of powder on every meal for the rest of the dog’s life, periodic B12, and a vet who checks in.

Practical notes owners find useful: buy enzymes in bulk once the dose is settled, since it is the main ongoing expense; keep a weight chart, because weight is the honest measure of whether the plan is working; and warn anyone who feeds your dog, including boarding kennels, that every meal needs the powder.

Why this breed, and what it means for breeders

EPI happens in other breeds, but the mechanism in German Shepherds is specific enough to be worth stating.

Merck: “pancreatic acinar atrophy is the most common cause of exocrine pancreatic insufficiency in German Shepherd Dogs, ‘rough’ Collies, and Eurasiers, whereas chronic pancreatitis is the most common cause in dogs of other breeds and in cats” (Merck Veterinary Manual, retrieved 2026-08-12).

The distinction matters. In most breeds EPI is the aftermath of repeated pancreatic inflammation, which is something that happens to the dog. In German Shepherds the usual route is atrophy of the enzyme-producing cells themselves, which behaves like an inherited predisposition rather than an accident.

For anyone buying a puppy, that makes it a fair question to ask a breeder: has EPI turned up in this line? There is no simple screening test comparable to a hip score, so the honest answer is family history rather than a certificate. A breeder who knows their lines will be able to tell you.

Questions worth asking your vet

  1. Can we run a TLI test? Ask by name. A general blood panel does not include it.
  2. What was the actual TLI number? Merck gives 5.5 mcg/L or below as diagnostic in dogs. The value indicates how much function is left.
  3. Have cobalamin and folate been measured? This is the one that predicts outcome.
  4. Powder or tablets, and why? Merck rates powders as more effective, and enteric-coated products as the least suitable.
  5. What starting dose, and when do we try reducing it? The aim is the lowest effective dose, which matters because this is the main ongoing cost.
  6. What diet do you want us on? Merck recommends low insoluble or nonfermentable fibre, which rules out some foods marketed as digestive.
  7. How often do we re-weigh and recheck? Weight is the honest measure of whether the plan is working.

If your dog is already on enzymes and still not thriving, take question 3 back to your vet before increasing anything else.
